Please use the forms and fees information below to provide us with the required information for services in 2026.
If the service is officiated by a stipendiary minister licensed to a Parish please put the Parish name on the form and mileage claims will be paid to the parish directly.
To ensure full functionality, we recommend opening and completing this form using Microsoft Excel. While the form may open in other spreadsheet software (e.g., Google Sheets, Apple Numbers), some features such as dropdown lists, conditional formatting, and data validation may not work as intended.

2026 Update - Next of Kin
From 01/01/2026, we will be updating the way next of kin funeral fees are administered.
The Parochial Church Council (PCC) will now be responsible for collecting all funeral fees directly from the next of kin.
Once the fees have been collected, the PCC should:
- Complete and submit a PF1 (choosing the invoice the PCC option)
- Await an invoice from the Diocesan Board of Finance (DBF)
- Remit the DBF element to the Diocese using the invoice number as the payment reference
The process will effectively work in the same way as the current PF2 process.
The DBF will no longer directly invoice the next of kin.
The DBF will continue to collect funeral fees from funeral directors for all other funerals.
